How to Obtain a Dress
Gowns and accessories are given away, free of charge, to girls who may not have the opportunity to attend Prom due to economic hardships, however, they must meet certain financial and scholastic guidelines. "Boutique Day" is held on a specific day prior to special events where girls may come and pick out the dress of their dreams.
Guidelines~
- Girls must be a junior/senior in high school, currently enrolled and present their Student ID Card or other proof of enrollment and posses no less than a "C" grade average.
- Girls must be referred to us by a teacher/counselor, church, or community leader. Extenuating circumstances are carefully considered.
- Girls will receive one dress, plus a related accessory if available
- Girls must be referred or have an appointment to attend the "Boutique Day".
~Please note, The Prom Fairy would love to help everyone however, dresses and sizes are limited; we cannot guarantee availability to everyone.
The referral form is below. Please return the form to us no later than March 31, 2010 You may scan and email the form back to us, fax the form to 702.896.7341 or email us with your name, school, and contact information.

Please contact us if you do not receive a confirmation email after sending in your referral. Contact Amme at 702.531.4353 to confirm that your fax was received.
The princess treatment~ What to expect at The Prom Fairy’s "Boutique" day:
- You will be able to select from our inventory of formal gowns guided by your own personal Fairy Godmother.
- Each student will be asked to wait until a Fairy Godmother becomes available before she can begin shopping. Please plan accordingly (i.e. bring something to read, don't make other appointments for that afternoon)
- *All items are free of charge and are yours to keep.
- Each student will be permitted to bring one person (i.e. a parent, guardian, etc.) with them into the boutique to help them shop with the volunteer.
- Adults will not be allowed to shop unless the student for whom they are shopping is physically present with them at the boutique.
- Please allow 1 hour for your appointment.
- Remember to bring your student ID or letter on school letterhead stating that you are a junior or senior. Without proof of grade and enrollment, we cannot keep your appointment.
- Princess' will be entered in a drawing to receive a free Prom beauty treatments.

*If you choose to do so, once your Prom is over, you can always donate your dress to help another student.
